Personal Care Services
Personal Care (PC) services are available to assist an eligible member to perform activities of daily living (ADLs) and instrumental activities of daily living (IADLs) in the member's home, place of employment or community.
What you get
Services, Coverage, as recorded:
- Members can receive a maximum of 210 hours of service per month based on assessed needs. Direct care workers (including SFCPs) are limited to billing no more than 16 hours per day.

How to apply

Fill out and sign a Personal Care-Medical Necessity Evaluation Request (PC-MNER) form and provide it to your doctor (MD or DO), physician's assistant, or registered nurse to complete, sign, and submit to Acentra Health-PC, 1007 Bullitt St., 2nd Floor, Charleston, WV 25301 (toll-free 1-866-385-8920, fax 1-844-794-6729). An Acentra Health registered nurse will then contact you to schedule a medical eligibility assessment in your home.
