Personal Care Services is a Wisconsin Medicaid benefit providing assistance with activities of daily living including bathing, toileting, feeding, dressing, mobility, and personal hygiene. How to apply

    The member's provider orders personal care services and a Medicaid-enrolled personal care agency's RN supervisor requests prior authorization by submitting the Personal Care Screening Tool (PCST), which can be filled out online at https://fsia.wisconsin.gov/ or on paper, based on an in-person evaluation of the member in their home.
