VA Home-Based Primary Care (HBPC)

Interdisciplinary VA primary care team — physician-led, including nursing, social work, mental health, rehabilitation, pharmacy, and dietitian — that delivers comprehensive primary care in the veteran's home. Designed for veterans too complex or frail for routine clinic visits. Includes caregiver education and support, directly reducing family caregiver transport and coordination burden.

What it offers

Home Based Primary Care provides VA health care services in the Veteran's home for Veterans who need team-based in-home support for ongoing diseases and illnesses that affect health and daily activities. Veterans often have difficulty making and keeping clinic visits because of illness severity and are often homebound, though VA says homebound status is not required. The program can also support Veterans who are isolated or whose caregiver is experiencing burden, and can be used with other Home and Community Based Services. Services may include primary care visits at home, care management, social work, rehabilitation, psychology, nutrition, and pharmacy. All enrolled Veterans are eligible if they meet clinical need and the service is available; a copay may be charged based on service-connected disability status and financial information.
