Ohio's Structured Family Caregiving is a Medicaid waiver service for waiver-enrolled individuals at least 18 years old in MyCare Ohio, Ohio Home Care, or PASSPORT who live with a caregiver providing daily care and support. Payment, provider, and financial eligibility rules vary by waiver pathway.

The service can be authorized through MyCare Ohio, Ohio Home Care, or PASSPORT. Income and asset eligibility are determined through the member's Medicaid or waiver pathway, not a standalone SFC income test.
