How to apply

To first become DDSN eligible (a prerequisite for IFS-R respite funding), apply by phone, toll free, at 1-800-289-7012, available 8:30 AM - 5:00 PM Monday through Friday. Respite funding itself is requested through the individual's DDSN case manager using the Attachment B application.
