State Plan Personal Care Services provides personal care assistance to individuals 18 years or older with physical or cognitive impairment requiring help with activities of daily living who are eligible for Medicaid or Healthier Oregon.

How to apply

An older adult or an adult with a disability eligible for or receiving case management services from the Department's Aging and People with Disabilities (APD) or Area Agency on Aging (AAA) must apply for State Plan personal care services through the local APD or AAA office. Medicaid long-term care benefits generally: By phone, call the ADRC at 855-673-2372; online, apply through ONE Online; in person, call your local Benefits office to make an appointment.
