1915(c) Aged and Physically Disabled Waiver
CMS-approved Medicaid 1915(c) waiver run by APD for aged and physically disabled adults. It funds waiver case management, one-time community transition set-up expenses for people leaving institutional or provider-operated settings for in-home care, and housing support services.

A Department or Area Agency on Aging case manager assesses service needs with the CA/PS assessment, determines eligibility, and offers service choices; the assessment calculates the service priority level
