    There is no single online PACE application; potential participants contact one of Oklahoma's four PACE organizations (Valir PACE-OKC, Valir PACE-Shawnee, Cherokee Elder Care PACE in Tahlequah, or LIFE PACE in Tulsa, chosen by service-area zip code) or the OHCA PACE Unit (PACEInquiry@okhca.org, 405-522-7044) for information; the PACE organization then helps the senior apply for Medicaid and gather the required financial/asset and income verification documents.
