Money Follows the Person
Money Follows the Person supports SoonerCare members aged 19 and older with disabilities in transitioning from institutional settings to community living, including frail elderly individuals 65 and older, individuals 19-64 with physical disabilities, and individuals 19 and older with intellectual disabilities.
What you get
Services, as recorded:
- Community transition services: up to $2,400 per person for essential goods/services; separately, transitional funds up to $3,000 to help facilitate the move
