Community First Choice
Community First Choice provides self-directed personal care services in the home for Medicaid-eligible individuals with health conditions requiring nursing home level of care, allowing adult children and relatives to be paid as personal care attendants.
What you get
Services, Coverage, as recorded:
- hours of authorized personal care are set every two weeks via a Service Profile (SLTC-155) based on individualized assessed need; PCA work is scheduled within authorized units in 15-minute increments

1) Apply for Medicaid at apply.mt.gov or call (888) 706-1535; 2) schedule an assessment with the Department's Utilization Review Contractor (URC) at (800) 219-7035 to create a Service Profile; 3) choose a provider agency; 4) the agency develops a Service Plan (SLTC-175) with you
