1. Make a referral to BSW by contacting Mountain Pacific at (800) 497-8322 or (406) 443-4020 (level of care screening). 2. County Office of Public Assistance eligibility specialists determine Medicaid eligibility. 3. Case Management Team assesses in person, places applicant on wait list (SLTC-146 criteria tool) when slots are full, then holds an intake meeting to develop the service plan.
