How to apply

1. Contact your local Area Agency on Aging to complete a preliminary level of care screening by telephone; if eligible, the AAA mails a waiting list placement letter. 2. Apply for Indiana Medicaid (AAA can assist; fssabenefits.in.gov). 3. When invited from the waiting list by letter, an AAA options counselor schedules an in-person full eligibility assessment. 4. Select a PathWays health plan through the enrollment broker (877-284-9294). Services begin once Medicaid and the waiver are approved and the service plan is complete.
