How to apply

Personal Care Services is one of the minimum covered services under every Florida Medicaid Managed Medical Assistance (MMA) and Long-Term Care (LTC) plan. There is no separate consumer application: the recipient must already be enrolled in Florida Medicaid (an MMA or LTC plan), be under the care of a physician, and have a physician's order for personal care services; the plan/provider then completes the required assessment and Plan of Care.
