Arizona Self-Directed Attendant Care — SDAC and Agency with Choice (ALTCS Managed Care)

Arizona Long Term Care System (ALTCS) members living at home may use member-directed attendant care options through ALTCS managed care. AHCCCS describes Agency with Choice as a shared-employer model and Self-Directed Attendant Care as a model where the member or legal guardian is the legal employer. SDAC allows many family, friend, or neighbor workers, but AHCCCS guidance excludes spouses, parents of minor children, and legal guardians from being hired under SDAC.

What it offers

AHCCCS says member-directed options are available to most ALTCS members who live in their own home and receive services such as attendant care, personal care, or homemaker services. Agency with Choice lets the member share employer responsibilities with an agency, while the agency retains authority to hire and fire and provides training. SDAC makes the member or legal guardian the legal employer, with a fiscal employer agent handling taxes, payroll, and paychecks. AHCCCS SDAC guidance says members may hire family, friends, or neighbors except spouses, parents of children age 18 or younger, and legal guardians overseeing care.
